چکیده

Background: The propyl derivative of the pipecoloxylidides was less toxic than the butyl derivative (bupivacaine). Further work revealed that the nerve blocking properties of the R and S enantiomers were similar but that the S-enantiomer was less cardiotoxic. Thus the S enantiomer of the propyl derivative (Ropivacaine) was chosen for further development.Ropivacaine, structurally resembling bupivacaine, with a propyl group on the piperidine nitrogen atom of the molecule is a relatively new amino-amide anaesthetic agent, similar in chemical structure to bupivacaine has been recently launched in India, for clinical evaluation having various advantages like early onset and shorter duration of action and having lesser cardio toxicity as compared to bupivacaine. The drug ropivacaine, relieves the psychological distress of being immobile for a longer period of time after lower abdominal surgeries. In view of the above context the present study was undertaken for comparison of isobaric ropivacaine and isobaric bupivacaine to determine clinical efficacy of ropivacaine. Materials and Methods: We conducted this study as a prospective randomized study. A total of 100 adult patients were randomly selected who fulfilled the inclusion criteria. They were divided into groups and study was conducted. Results: In this study,we measured the results of each drug by its action on onset and peak of sensory and motor blockade. Conclusion: From our study we conclude that both Bupivacaine and Ropivacaine with 25 mcgms of fentanyl intrathecally, promote satisfactory anaesthesia for lower limb surgeries. The spinal anaesthesia with intrathecal Ropivacaine 15 mg provides a faster motor recovery as compared with Bupivacaine 10 mg which is more suitable for ambulatory lower extremity surgeries of approximately two hours.

Haemodynamic Comparison of Intrathecal Hyperbaric 0.5% Bupivacaine 2ml(10mg) with Isobaric 0.75% Ropivacaine 2ml(15mg) with Fentanyl 20μg as Adjuvent in Geriatric Patients undergoing Major Lower Limb Orthopedic Surgery

Aims and Objectives: Spinal anaesthesia is the most commonly used method of anaesthesia and analgesia in lower limb surgeries. In elderly patients spinal anaesthesia causes more sympatholysis and hemodynamic disturbances than young patients. So low dose of local anaesthetic is preferred which may occasionally cause failure of spinal anaesthesia .Therefore, different adjuvents are added to achie...

Effect of Intrathecal Fentanyl on subarachnoid block with 0.5% hyperbaric bupivacaine

Background and objectives: Subarachnoid block is most versatile regional block commonly employed for infraumbilical surgeries. The use of neuroaxial opioids has gained widespread popularity as they potentiate the analgesia produced by local anesthetics. Hence the present study was undertaken to evaluate the effects of intrathecally administered fentanyl (25mcg) on the characteristics of hyperba...

Effects of Adding Intrathecal Magnesium Sulphate To Bupivacaine And Fentanyl in Lower Abdominal And Lower Limb Surgeries

Background & Aim: Addition of opiates with nonopioid adjuvants to local anaesthetics is becoming popular in perioperative pain management, as no single agent has yet been identified to specifically inhibit nociception without the usual side effects. Research continues concerning different techniques and drugs that could provide optimal operative conditions and postoperative pain relief. The pre...

Objective: To compare the block characteristics and haemodynamic stability of intrathecal Ropivacaine and Levobupivacaine with Fentanyl and Magnesium as adjuvants for lower abdominal surgeries. Method: Sixty patients of ASA grade I and II coming for elective lower abdominal surgeries under spinal anaesthesia were randomly allocated to two groups with 30 patients in each group. Efficacy and Appropriate Dosage of Isobaric Ropivacaine for Spinal Anesthesia in Patients Undergoing Elective Lower Limb Orthopaedic Surgeries

Background: The dose response of ropivacaine has not been extensively determined yet. This study was conducted to estimate minimum effective local anaesthetic dose and to assess the duration of sensory and motor block, and side effects if any, of intrathecal administration of ropivacaine for lower limb surgeries. Materials and Methods: 120 patients aged between 20 years and 60 years of either s...
